• Please review and print the Services Agreement.(pdf format)*
• Sign the agreement on Page 4 and initial the Fee Schedule and Additional Services Addendum on Page 5.
• Complete and initial the Business Online Banking Set-Up Addendum on Page 6.This must be completed when enrolling for any of the online banking services.
• If you are interested in Bill Pay, Wire Transfers, or other functions available through our Business Online Banking system, check the appropriate boxes on the Fee Schedule and Additional Services Addendum on Page 5. A customer service representative will contact you regarding set-up.
